Chemical Dependency Assessments

I have more than fifteen years experience performing chemical dependency assessments ordered by: The Courts, State and Federal Probation; Employee Assistance Programs; and by individual request.
There are many reasons for an individual to schedule an appointment for a chemical dependency assessment, such as:

• DUI
• Petition for Deferred Prosecution
• Domestic Violence Charge
• A condition for probation
• Child Custody Case
• A condition for pre-employment due to a positive drug test
• A condition for continued employment due to a positive drug test
• Worry about your alcohol or drug habits

I will make every effort to schedule an appointment within five working days after the day you schedule your first of two appointments required to complete the assessment.
